How to Make a Referral
1. Complete Our Referral Form
Start by filling out our secure online referral form.
2. Send Supporting Documents
To begin the intake process, please include:
- Diagnostic evaluation
- Insurance card (front and back)
- Referral form (if required by insurance)

3. We’ll Take It From There
Once your referral is received, our intake team will contact the family within 1 business day and begin walking them through the next steps with clarity, care, and compassion.
